DMV FORMS:
DMV ONLINE
Accident reports:
Provided to you by the Oregon DMV to complete within 72 hours of an accident that is required to be reported. These forms can be found on the front and back of our building and are obtainable 24/7. Per Oregon law, they must be returned to any DMV by mail or in person. Envelopes are provided for your convenience.
Change of address forms:
Per Oregon Law, you have 30 days to change the name or address on your Oregon driver's license or id card. These forms are located inside our lobby which is open during normal business hours.
Driving Manuals:
We have Oregon driver's manuals, commercial driver's manuals, motorcycle driver's manuals, and teen driving manuals. Theses are located inside our lobby which is open during normal business hours.

FINGERPRINTING:
The Molalla Police Department offers fingerprint services to the public. This service will be available Tuesday - Thursday from 9:00 am – 3:30 pm. Due to the time and staffing involved, an appointment scheduled in advance is required. Services will only be available to residents of or for employment in the 97038 zip code. There is a $20.00 fee per fingerprint card. Government photo I.D. is required. For more information visit our site; www.cityofmolalla.com/police, or give us a call at 503-829-8817. EXTRA PATROL/AREA COMPLAINTS:
Extra Patrol Request
If you are having a concern or problem with a specific area during a specific time, or have concerns about a house with lots of suspicious foot traffic, this form is what you need. Fill this form out with the most specific location, time of day, day of week, and other info as you can and we will make an attempt to focus some extra patrol on it. Don't write "speeders at all times of day and night". Instead, write "vehicles speeding around 3pm and 6pm on weekdays".
RIDE-ALONGS

Citizen's interested in learning more about law enforcement and want to see a police officer at work can turn in a request form, and based on a quick criminal history check qualification, can go out with one. If you are thinking of becoming an officer or are just curious, the experience is quite unique. One ride-along per person, per year.
VACANT RESIDENT CHECKS
Vacant Residence
If you are going on vacation or going to be away for an extended time-period, you can fill out a request form and we will attempt to keep an extra eye out while you are gone. Of course, it's not a guarantee of security, but it might help. Observant neighbors are your best bet for noticing out-of-the-ordinary circumstances.
RADAR TRAILER REQUESTS

Upon request, we can place the radar speed observation trailer at any location inside the city limits. This trailer was donated to us and is maintained and placed by a generous volunteer. It does not take photos, nor can it be used for issuing citations. It is to make driver's aware of their speeds in high complaint areas.
ALARM PERMITS
There is a permit required to have an alarm for your home or business. Once you submit your application a permit will be issued and renewal notices sent out annually. There is no fee for 65+ or government facilities. The initial and annual fee for a residence is $30. The initial and annual fee for a business is $50. PUBLIC RECORD REQUESTS
Ride-Along Application
If you want a copy of a police report, you need to fill out and submit a records request form. Based on current public record laws in Oregon, there are many situations when a record is NOT public record. For instance; any current court case (ticket/arrest/under consideration of charges), any report generated by another agency, or anything containing personal information will not be available to the public. There is a fee for a copy of a case. There is never a fee for the victim. A police report is not the same as getting a criminal history. If you need to obtain a criminal history/record you must contact the Oregon State Police.
SEX OFFENDER REGISTRATIONS

Registrations are done on Monday - Friday, during business hours, for Clackamas County residents only.
We perform sex offender registrations for those needing to do a change of address or annual renewals. 10 days before or after the date of change is the requirement for re-registering. Please have photo i.d. and all required information, or you will not be allowed to complete your registration. These are only done by the reception staff.
LOST/FOUND PROPERTY

We release property by appointment only.
When someone turns in found property to us, we try to locate the owner and notify them. If you have lost items, please report them and check with the property officer to see if they may be here. After legal periods of advertisement and/or notifications, all property is considered abandoned.
YOUTH MINI-SCHOLARSHIPS
Youth Fund Application
All the proceeds from property that becomes abandoned by law is put into the youth scholarship fund. A parent or guardian can apply for one scholarship, per child, per school year. If you have more than one child that each do an activity, you can apply for one for each student. The student needs to be within the Molalla River School District Area.
