Juror Questionnaire

Attached is a fillable version of the Juror Questionnaire. You must know your juror number and have it stated on the form before submitting to the Court. If you do not know your juror number, please call the Court at (503) 829-7711. Because a signature is required, the form is fillable, however it will need to be printed and signed before submitting to the court.
